Redesign for Community-Based Organizations 

This package is designed to help community-based organizations shift towards a community health management approach: improving population health, enhancing the experience of care, and reducing per capita healthcare costs. By focusing on populations rather than individuals, we aim to improve health outcomes by addressing the broader social determinants that significantly influence well-being.

Research, including initiatives under the HITECH Act and the Affordable Care Act, indicates that social determinants contribute to approximately 20% of unimproved health outcomes. Our comprehensive approach includes:

• Prioritizing Health Informatics, including Clinical, Public Health, and Population Health Informatics
  
  
• Utilizing standardized screening tools for Health-Related Social Needs (HRSN) to assess social determinants of health
  
  
• Collecting and analyzing publicly available data to improve patient care quality
  
  
• Addressing factors beyond medical care, such as community involvement, food security, transportation, safety, and utilities, which significantly impact population health outcomes
  
  
• Bridging the gap between Public Health and Clinical care to provide integrated, holistic care solutions.
